This website is for professional medical-legal inquiries to Crux Neurology, PLLC. It is not a patient portal.

Information you submit

The inquiry form requests your name, organization, professional email, request type, and any optional contact or case information you choose to provide. This information is used to review your inquiry, consider whether the matter fits the practice, and communicate with you.

Do not submit patient names, medical records, sensitive personal information, or confidential case details through this form. Records should be shared only after an appropriate process has been agreed upon.

Form processing

Form submissions are processed by Formspree and routed to the practice's intake email. The website does not create an account for you or automatically send a CV, fee schedule, or other materials. Each request is screened.

Technical services

The website's hosting provider and form provider may process technical information needed to deliver and protect their services, such as connection and request information. Their handling of that information is governed by their own policies.

This website requests typefaces from Google Fonts. Those requests connect your browser to Google's font services. The page remains usable with system fonts when those services are unavailable.

Analytics and advertising

This version of the website does not include advertising pixels, session recording, marketing analytics, or first-party tracking cookies. Third-party service settings should be reviewed separately.
